This survey is to assist the CTAA in evaluating its athletic programs and developing its coaches.  Please take a few minutes to fill out this survey honestly.  Constructive criticism is welcome, as well as any suggestions and new ideas you may have to improve the CTAA.
This survey will be used as a tool to identify opportunities for improvement.  All complaints will be kept strictly confidential and will be used in accordance with our Coach Selection Policy.
