Application for Dog License

Please fill out the information below by using your computer mouse or tab button.  When complete mail a copy of the current rabies vaccination certificate, the correct fee (checks made out to Chesterfield Township) and the form to:

(Please note:  If the rabies certificate expires during the 2008 calendar year, you must obtain an updated rabies shot before the dog can be licensed.)

Fees for Dog Licenses are:
Spayed or Neutered Dogs:  $8.20
Non-Spayed Dogs:  $11.20
Late Fee:  (after March 31):  $5.00 per month
