You may fill this form out on your computer but it cannot be emailed to the Registrar because their is a fee involved.  When filling out this form, use the mouse/tab buttons to complete.  When the form is complete, you can print it from your computer then it can either be mailed to the Registrar of Vital Statistics, Bonnie Haines,  300 Bordentown-Chesterfield Rd, Chesterfield, NJ  08515 or you may take it to the Municipal Building with the required $10.00 per record fee.  Checks are to be made payable to Chesterfield Township.
