STATE OF NJ FIREARMS PURCHASER IDENTIFICATION CARD
ORIGINAL – For Rifle or Shotgun.  Also necessary for a PERMIT TO PURCHASE HANDGUN.
DUPLICATE – If your card is lost, mutilated or stolen and originally issued in Chesterfield Twp.
For a change of address, name, etc., from another issuing authority, if you now reside in Chesterfield Twp.
You must be 18 years old to apply.

PERMIT TO PURCHASE HANDGUN
This may or may not be requested by you with the application for Original Firearm ID Card.  You must be 21 years of age to apply.  If you wish a Permit to Purchase and you already have a valid NJ Firearms Purchaser Identification Card, you will only need a Request for Criminal History form (yellow sheet) and a Mental Health Consent Form.  NOTE:  Permits to Purchase are valid for 90 days from the date of issue.  This permit can be extended for only ONE additional 90-day period.  Please do not ask us to extend beyond that, we cannot without doing a Criminal History form again. 

INSTRUCTIONS

1.  You must be a resident of Chesterfield Township to apply.
2.  Fill out all paperwork COMPLETELY and PRINT LEGIBLY.
3.  Be sure that the address that you put on these forms matches your NJ drivers license address exactly.
a.  Be sure that ALL blocks are completed.  Place N/A in the blocks where questions do not apply to you.
b.  Answer all questions with YES or NO.  Do not circle answers.
c.  Make sure you sign all forms where necessary.
d.  You must supply two people that will be a reference for you.  These references must have known you for 2 years, be of good moral character, must not be a relative of yours or live in the same residence as you.  Please supply complete name, home mailing address (including zip code) and home telephone number.
e.  Supply your place of employment where asked and include the full name and address of your superior.

4.  You must obtain a CERTIFIED CHECK or MONEY ORDER made payable to  “DIVISON OF STATE POLICE – SBI” in the amount of $60.25 for fingerprint processing.  If you already have a valid SBI number and you only need a Request for Criminal History (yellow sheet), the amount is $18.00.  You must obtain a CERTIFIED CHECK or MONEY ORDER made payable to “DIVISON OF STATE POLICE – SBI”. (NJ State Police will NOT accept personal checks or cash.)              

5.  Please be aware that if you are changing your address on your Purchase Identification card, we will be sending a letter to the Original Issuing Authority to have all previous paperwork sent to our office.  Nothing can be issued until we receive this paperwork.

6.  When you have all paperwork completed, call 291-0912 to set up an appointment to be fingerprinted.  Please bring all paperwork and money order/certified check with you.  We will also make a copy of your driver’s license at this time, if we have not done so already. 

7.  The average waiting time to get results from the State and the Federal Government is now about 3 months for fingerprints.  For Criminal History check it is about 4 to 5 weeks.

8.  You will be contacted as soon as the results come back to this office and we will set up a date and time to finish your paperwork.
