Please complete all of the information on this form as it applies to your project. Submit this form with a survey of your property.  On the survey, draw the project you are requesting approval for and indicate on the survey the distances from the project to all property lines.  In accordance with the requirements of Township Ordinance 2006-16, if your parcel is less than one (1) acre in size, you are required to submit a grading plan indicating the existing grade of the lot, and the proposed/resultant grading of the lot following the construction and/or installation of the project you are requesting approval for.   There is a fee of $50.00 required to be submitted with this application.
